Senior Services Information & Referrals
The City of Pacifica's Senior Services Information and Referral program is available Monday through Thursday from 10:30 am to 1:00pm at (650)738-7352 to provide community linkages on aging issues.
San Mateo County provides the TIES Line to connect seniors to needed services and serves as the main entry point for adult services including Adult Protective Services. (800)675-8437 or TTY (800)994-6166.
The Community Information Handbook, which is updated and published annually by the County of San Mateo Human Services Agency and available online, is a comprehensive resource providing contact information for all human and social services in San Mateo County.

- Operation Brown Bag - Second Harvest Food Bank's Brown Bag program provides bi-weekly nutritious groceries to senior individuals (ages 60+) and disabled individuals (ages 55+) at distribution sites in Santa Clara County and San Mateo County. In Pacifica, the Brown Bag program is available on the 1st and 3rd Friday of every month at 9am at St. Peter Catholic Church. To enroll in the program, please visit St. Peter's one Friday morning and ask for Thelma or JoAnn. For more information, contact Second Harvest Food Bank at 650-610-0800.
- Produce Mobile - Second Harvest Food Bank delivers produce on the 2nd Monday of every month to the Pacifica Baptist Church from 12:30 - 4:00 pm. To be served at the Produce Mobile, you must self-certify that your household meets income guidelines based on the Federal Poverty Level. To formally enroll in the program, please contact the Pacifica Resource Center.

- Shopping Assistance for Seniors Homebound (SASH) - The Pacifica Senior Services SASH program is available for seniors who, due to limited mobility, cannot shop for themselves or do not have anyone to shop for them. A SASH volunteer will pick up your shopping list and money to purchase items and return with your receipt and groceries. - Tracie Dalton, RD, CDE - Registered Dietician Tracie Dalton provides nutrition analysis, education and counseling at the Senior Center, in Senior Center print materials, and during visits to homebound seniors. Please contact Information and Referral Coordinator at 650-738-7353 for more information or to schedule a consultation with Tracie.

- Affordable Housing Resource Packet
- California Registry - California Registry is a free service offered to seniors or their family members to assist them in locating retirement homes and assisted living in California.
- BRIDGE Housing - BRIDGE Housing strengthens communities by developing, owning and managing high-quality, affordable homes for working families and seniors.
- Eden Housing - Eden Housing builds and maintains high-quality, well-managed, service-enhanced affordable housing communities that meet the needs of lower income families, seniors, and persons with disabilities.
- HIP Housing - HIP Housing is a non-profit organization that creates a place to call home for thousands of people in San Mateo County. HIP Housing's Home Sharing Program fosters living arrangements among two or more unrelated people in which home owners or renters (Home Providers) who have a residence with one or more bedrooms are matched with persons seeking affordable housing (Home Seekers).

- Non-Profit Housing Association of Northern California - The Non-profit Housing Association of Northern California is the collective voice of those who support, build and finance affordable housing. We promote the proven methods of the non-profit sector and focus government policy on housing solutions for lower income people who suffer disproportionately from the housing crisis.
- Housing Leadership Council of San Mateo County - The Housing Leadership Council of San Mateo County works with communities and their leaders to produce and preserve quality affordable homes.

- Legal Aid Society of San Mateo County - The Legal Aid Society of San Mateo County's HomeSavers Project helps tenants who are having problems with their landlords. LAS offer legal counseling, advice and representation in the following areas: evictions, disputes over the condition of premises, and illegal acts by landlords.
- Project Sentinel - Project Sentinel is a non-profit organization. It's primary function is to assist individuals with housing problems such as discrimination, mortgage foreclosure and delinquency, rental issues including repairs, deposits, privacy, dispute resolution, home buyer education, post purchase education and reverse mortgages.

HICAP (Health Insurance Counseling and Advocacy Program) is a state and federally funded program that provides free, unbiased counseling and community education on Medicare and related health insurance issues. We provide one-on-one counseling by telephone appointment, and make Medicare presentations throughout San Mateo County.
HICAP counselors are registered by the California Department of Aging and help Medicare beneficiaries understand their options and make informed decisions about their healthcare. HICAP program staff and volunteers DO NOT sell, endorse or recommend any specific insurance companies, plans or products. Counselors help explain health insurance benefits, rights, and options, Medicare Part D (prescription drug coverage), Medicare Part C (Medicare Advantage Plans), supplemental insurance (Medigap), Medicare Savings Programs, long term care insurance, and Medicare billing, complaints and appeals, and Medi-Cal as it relates to Medicare (dually eligible beneficiaries). Medicare - Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).
- Medi-Cal - Medi-Cal is free or low-cost health coverage for children and adults with limited income and resources. Medi-Cal covers low-income adults, families with children, seniors, persons with disabilities, children in foster care as well as former foster youth up to age 26, and pregnant women.
- CareAdvantage - CareAdvantage is a Health Plan of San Mateo program that combines your Medicare and Medi-Cal benefits in one simple plan. This plan offers lots of additional benefits, like free taxi rides to approved medical appointments, vision care, and coverage for different types of long-term care services. You need to have Medicare Parts A & B and full-scope Medi-Cal to qualify. You must be a resident of San Mateo County. People with ESRD cannot apply (end-stage renal disease). People with developmental disabilities are eligible to apply.

- Center for Independence for Individuals with Disabilities (CID) - CID's mission is to provide support services, community awareness, and systems change advocacy to promote full and equal community integration and participation for people with disabilities in San Mateo County.
- City of Pacifica Special Needs Registry - The Special Needs Registry (SNR) is a tool that allows the police department to better serve our community members with special needs. It will allow officers quick access to information that will enable them to better assist that person during any contact, if they become lost, found, or when they need assistance.
- Fall Prevention Coalition of San Mateo County - Informs, collaborates and raises awareness to prevent falls among older adults living in our community. Provides resources and referrals for older adults, caregivers and service providers.
- Healthy Seniors, Healthy Pets - If you are a current City of Pacifica Meals on Wheels client and you have a pet, you may be eligible for the "Healthy Seniors, Healthy Pets" program which provides MOW clients with food and some types of care for their pets. This program is funded by a generous grants from the Banfield Charitable Trust and Meals on Wheels America. For more information, please contact Stephanie Coy, MOW Coordinator.
- Help at Home Directory - The Help at Home Directory is a mini-reference guide to assist San Mateo County adults of all incomes to remain in their homes when they are ill, disabled or in crisis. It is available in English, Chinese and Spanish.

- Home Safety Services - Installs grab bars, stair railings, wheelchair ramps and other safety/accessibility devices.
- In-Home Supportive Services (IHSS) - The In-Home Supportive Services (IHSS) program provides homecare services to Medi-Cal eligible aged, blind or individuals with disabilities, including children, to assist them to remain safely in their own homes as an alternative to out-of-home care.
- Institute on Aging - A San Francisco,CA-based non-profit (serving San Mateo County) dedicated to preserving the dignity, independence, and well-being of aging adults and people living with disabilities. Services include: Friendship Line, Home Care & Support Services, Social Day Program, Psychology & Counseling Services and more.
- Pacifica Library Homebound Service - The Pacifica Sanchez and Sharp Park Libraries are pleased to provide a Homebound Program. This program provides services to those who are unable to get to the library because of short or long-term illness, non-driver status (senior), physical challenges, or visual disabilities. Program benefits include personalized visits, no overdue fines, and longer lending periods. Library staff can deliver books, movies, magazines and audio books right to your home.

- Palliative Care at San Mateo Medical Center
- Rebuilding Together - Through volunteers and donations, Rebuilding Together makes repairs that improve the health, safety, and independence of our neighbors. Rebuilding Together serves senior citizens, single parents, grandparents raising grandkids, people with disabilities, families with children, veterans, children caring for their ailing parents.
- San Mateo Support Staying Home - Do you live alone with memory problems in San Mateo County? Would you like to learn ways to maintain independence and connect to community support? The Alzheimer's Association, in partnership with the County of San Mateo, is offering a free program to assist persons with memory problems who live alone. If you have memory problems, including mild cognitive impairment, early Alzheimer's, dementia, or other cognitive impairment, we would like to work with you to help you stay in your home for as long as possible. - Weatherization Services - HEAP applicants may be eligible for Weatherization Services. Energy efficiency-improving measures are done in the home to reduce energy costs. This program is available to renters as well as homeowners.
- Kara Grief Support - Grief support for children, teens, families and adults.
- Friendship Line - Our connections to one another are what bind us to life, yet for isolated individuals and their often equally isolated caregivers, these connections tend to disappear. The Friendship Line at Institute on Aging is the nation’s only 24-hour toll-free hotline for older and disabled adults. Trained staff and volunteers make and receive calls to and from individuals who are either in crisis or just in need of a friend.

- National Alliance on Mental Illness (NAMI) - NAMI San Mateo County is dedicated to improving the quality of life for people with mental illness and their families through support, education, and advocacy.
- Personalized Mental Health Services for Seniors - Mills-Peninsula offers specialized and compassionate care for seniors who are struggling with depression or other mental health issues. We can help you overcome issues including: depression; anxiety; mental health issues related to difficulty in coping with loss of a spouse or your job, retirement, reduced income, isolation, and medical problems addiction to drugs, alcohol, and prescription medications; both psychiatric/mental health and substance abuse problems (dual diagnosis). Our mental health services for seniors focuses on the needs of the individual, with assessment and treatment plans tailored to your needs and include intensive outpatient therapy to help you overcome mental health issues and/or substance abuse problems, as well as Transitions After Care group therapy to help you cope with the challenges of aging and return to the activities you enjoy.

- San Mateo County Behavioral Health and Recovery Services - Behavioral Health and Recovery Services (BHRS) offers a broad spectrum of mental health and substance use services for children, youth, families, adults and older adults in San Mateo County in the prevention, early intervention and treatment of mental illness and/or substance use conditions. BHRS provides mental health services to individuals who are eligible for Medi-Cal and members of the Health Plan of San Mateo.

- Senior Peer Counseling - To help our region’s older adults stay active and engaged, Peninsula Family Services' Senior Peer Counseling program offers an innovative peer-to-peer counseling program in English, Mandarin, Tagalog, and Spanish, as well as individualized services for lesbian, gay, bisexual, and transgender participants.
- San Mateo County Older Adult Mental Health Services (OASIS) - OASIS serves seriously mentally ill patients, home-bound, and on Medi-Cal who are 60+ in San Mateo County.
- Adult Protective Services - APS Case managers investigate situations where seniors or vulnerable adults have been subjected to, or are at risk of, abuse, neglect or exploitation. APS case managers ensure the safety of the person at risk and work with the individual to develop an appropriate care plan.

- Legal Aid Society of San Mateo County - Legal Aid provides free civil legal services to low-income San Mateo County residents. Our clients are the elderly, families trying to make ends meet, persons with disabilities, children with special needs, survivors of domestic violence, and people struggling to find health care. Our work includes five broad areas: obtaining health care, preventing homelessness, accessing income support programs, fighting for freedom from domestic violence and bridging the education gap. - Legal Aid Society of San Mateo County - The Legal Aid Society of San Mateo County's Senior Advocates Program serves residents age 60 and over in San Mateo County offering legal advice, advocacy, and representation in the following areas: government benefits, consumer law and debt collections, estate planning and simple wills, guardianships for seniors caring for young children, and advance health care directives.
- Season of Sharing - The Chronicles' Season of Sharing Fund offers grants to help families and seniors in crisis so they can pay for housing and emergency needs. Grant monies from the Fund can help families or seniors in any number of ways, such as paying the rent so they can stay in their home, assisting with delinquent utility bills, covering a security deposit so they can transition out of a homeless shelter, or buying a wheelchair or other essential medical equipment that they couldn’t otherwise afford. Pacificans can contact the Pacifica Resource Center to determine if they are eligible for Season of Sharing funds.
- St. Vincent de Paul Society - The Society of St. Vincent de Paul of San Mateo County provides basic survival necessities, offers nourishment to the hungry and distributes critical financial support and resources. Pacificans can contact the Pacifica Resource Center to determine if they are eligible for a referral to the St. Vincent de Paul Society for assistance with delinquent rent or utility bills.

- Kinship Support Services Program - Family members often step up to fill the gap when crisis, substance abuse, mental illness, incarceration, economic hardship, divorce, domestic violence, or other challenges take parents from a child. These courageous "kinship caregivers" can get the help they need through the Edgewood Kinship Support Services Network (Edgewood Kinship).
- Pacifica Youth Service Bureau - The Youth Service Bureaus are one of those valuable resources you can use if you find your family in need of talking, working out a problem, or in crisis. We have been a community resource since 1969. We offer a complete array of clinical mental health, school-based prevention programs, and family support services.
- StarVista - StarVista delivers high impact services through counseling, skill development, and crisis prevention to children, youth, adults and families.
- San Mateo County Commission on Aging - In accordance with the Older Americans Act and Older Californians Act, the San Mateo County Commission on Aging advises the Board of Supervisors and Aging and Adult Services (AAS) in its role as the Area Agency on Aging for the county and advocates and supports services for older adults through out the County.
- California Department on Aging - The California Department of Aging (CDA) administers programs that serve older adults, adults with disabilities, family caregivers, and residents in long-term care facilities throughout the State. The Department administers funds allocated under the federal Older Americans Act, the Older Californians Act, and through the Medi-Cal program
- Ombudsman Services of San Mateo County - OSSMC challenges long-term care facilities to deliver the highest standards of individualized care for their residents. We advocate for the health, safety, and dignity of these residents and broader changes in the system.
